IF YOU ARE A PROVIDER LICENSED BY THE DBHDS, YOU HAVE A LEGAL OBLIGATION TO ENSURE THAT THAT EVERYONE THAT YOU SERVE RIGHTS ARE PROTECTED AS STIPULATED IN 12VAC35-115-50. DIGNITY. THERE ARE TIMES WHEN THE INDIVIDUALS THAT YOU SERVE ARE IMPACTED IN A NEGATIVE WAY. THIS IMPACT MAY INCLUDE PHYSICAL, SEXUAL OR PSYCHOLOGICAL ABUSE. THE INDIVIDUAL MAY HAVE THEIR CIVIL LIBERTIES IMPACTED IN A WAY THAT THEIR WANTS AND DESIRES ARE DISALLOWED. THE INDIVIDUAL MAY BE EXPLOITED. 

When anyone who works for a provider identifies an issue, their role is to report it. Once the issue is reported, the providers’ role is to investigate it. This involves finding out the root cause of the incident and determining whether the allegation is substantiated or unsubstantiated.
